ABSTRACT

Objective@#To explore the antimicrobial effect of berberine combined with vancomycin on isolated Clostridium difficile (CD) strains in vitro, in order to provide some new ideas for the treatment of CD-related diseases.@*Methods@#Thirty-five clinical isolated CD strains were selected for single or combined susceptibility tests of vancomycin (concentration range 0.062 5~4 mg/L) or berberine (concentration range 8~512 mg/L) to CD by agar dilution method , the drug sensitivity result of single and combined drugs were analysed.@*Results@#All 35 CD strains were sensitive to berberine and vancomycin, where the sensitivity ranges of berberine and vancomycin were 128 mg/L to 512 mg/L and 0.25 mg/L to 2 mg/L, respectively. The 50% minimum inhibitory concentration (MIC50) values were 512 mg/L and 0.25 mg/L, and the 90% minimum inhibitory concentration (MIC90) values were>512 mg/L and 1 mg/L, respectively. Different concentrations of berberine combined with 0.062 5-4 mg/L vancomycin respectively, the strongest synergistic effect was 128 mg/L berberine combined with 1 mg/L vancomycin (accounting for 5.71%), the strongest additive effect was 64 mg/L berberine combined with 0.5 mg/L vancomycin(accounting for 25.71%), the strongest irrelevant effect was 8 mg/L berberine combined with 0.0625 mg/L vancomycin (accounting for 97.14%), no antagonistic effect of berberine and vancomycin was found (χ2=9.85, P=0.003).@*Conclusions@#Berberine can increase the antimicrobial activity of vancomycin against CD, and has the synergistic value for clinical medication.

Objective@#To test the differences in diffusion properties-namely fractional anisotropy (FA) and apparent diffusion coefficient(ADC)-between amateur marathon runners and healthy volunteers′ thigh muscles using diffusion tensor imaging (DTI) at rest.@*Methods@#Thirty amateur marathon runners(Group A) and 20 healthy volunteers (Group B) were recruited. All were males and their age was between 28 to 53 years. All subjects underwent both thigh DTI examination. DTI parameters FA and ADC were evaluated in thigh muscles(rectus femoris, vastus lateralis, vastus intermedius, vastus medialis, adductor longus, adductor magnus, biceps femoris long head, semitendinosus and semimembranosus)in all subjects. We tested the differences in diffusion properties between GroupA and B using independent sample t test.@*Results@#The average FA value of the thigh muscles measured in Group A was (0.24±0.03), which was significantly lower than that in Group B=(0.27±0.03)(t=5.164,P<0.01). The average ADC value of the thigh muscles measured in Group A was (1.74±0.10)×10-3mm2/s, which was significantly higher than that in Group B=(1.70±0.09)×10-3mm2/s (t=2.060,P<0.05). The FA value of Group A was lower than that of Group B in the rectus femoris, vastus lateralis, vastus medialis, biceps femoris long head, semitendinosus and semimembranosus (P<0.05). The ADC value of Group A was higher than that of Group B in the vastus intermedius, vastus medialis and biceps femoris long head (P<0.05).@*Conclusions@#In the absence of positive findings from conventional MRI, DTI technique can non-invasively assess the subtle changes in the bilateral thigh muscles of amateur marathon runners at rest in the early stage of microscopic molecular level, providing important imaging evidence for early changes in skeletal muscle caused by marathon.

Objective@#To study the effects of half marathon loading on the T2* mapping assessment of knee joint cartilage.@*Methods@#Twenty male amateur marathon runners with more than three years of running age were enrolled. The T1WI, PDWI, 3D-DESS, and T2* mapping were performed on right knee joint before and after the half marathon within 3h and after a period of approximately 1 week. Cartilage was divided into 6 regions: medial and lateral femoral cartilage, medial and lateral tibial cartilage and patella and trochlea cartilage. Femoral cartilage was partitioned into central weight-bearing regions, posterior cartilage nonweight-bearing regions with the posterior boundary of the meniscus as the dividing mark. All cartilage was further divided into deep and superficial layers according to the thickness 1/2, and the ROI was delineated to obtain the T2* value of the corresponding area. One-way repeated measures analysis was performed to test the difference of T2* values pre- and post-marathon. LSD tests were used to estimate the different influences of physiological activities on each articular cartilage area.@*Results@#All cartilage areas of the knee joint did not show cartilage injury during the examination. The differences in the global T2* values of the superficial layers of the knee cartilage before and after the half-marathon marathon were statistically significant (t=5.145, P<0.05), but the global T2* value of the deep region were not (t=1.864, P>0.05). After a week, the T2* values of all cartilage areas were not statistically different from those before running (t=1.062, 0.309; P>0.05).@*Conclusion@#After a single half marathon, the changes of knee cartilage composition in male amateur marathon runners are reversible. In the absence of positive findings in conventional MRI, T2* mapping can indirectly reflect changes in biochemical composition of the articular cartilage during exercise.

ABSTRACT

To evaluate right atrial ( RA ) structure and function of different left ventricular geometry in patients with obstructive sleep apnea syndrome ( OSAS ) by real‐time three‐dimensional echocardiography ( RT‐3DE) . Methods One hundred and ninety‐nine patients with habitual snoring were diagnosed as OSAS by polysomnography with apnea hypopnea index ≥ 5/h . Fifty gender ,age matching healthy people were selected as control group . Blood pressure and general clinical characteristics were collected and echocardiography was performed next morning . On the basis of left ventricular mass index , relative wall thickness ,left ventricular end‐diastolic diameter ,patients were divided into 6 groups :normal geometry ( NG) ,concentric remodeling ( CR) ,eccentric non dilated hypertrophy ( ND‐EH ) ,concentric non dilated hypertrophy ( ND‐CH ) ,eccentric dilated hypertrophy ( D‐EH ) ,and concentric dilated hypertrophy ( D‐CH) .Full volume images at apical four‐chamber view were collected . Right atrial volume‐time curve , RA maximum volume ( RAVmax ) , RA minimum volume ( RAVmin ) , RA pre‐contraction volume ( RAVpre‐a) were analyzed with QLab workstation . Following parameters including RA total emptying volume ( RA TotEV ) , RA total emptying fraction ( RA TotEF ) , RA passive emptying volume ( RA PassEV ) ,RA passive emptying fraction ( RA PassEF) ,RA active emptying volume ( RA ActEV ) ,RA active emptying fraction ( RA ActEF) were calculated . All structural parameters were corrected by BSA . Results①RA structural parameters :compared with control and NG groups ,RAVmax ,RAVmin ,RAVmin/BSA , RAVpre‐a and RAVpre‐a/BSA were increased in ND‐EH ,ND‐CH and D‐( CH+ EH) groups ( P <0 .05) . Compared with control ,NG and CR groups ,RAVpre‐a and RAVpre‐a/BSA were increased in ND‐EH ,ND‐CH and D‐( CH + EH ) groups , RAVmax , RAVmax/BSA , RAVmin , RAVmin/BSA , RAVpre‐a and RAVpre‐a/BSA were increased in ND‐CH and D‐( CH+EH ) groups( P <0 .05) . Compared with control , NG ,CR and ND‐EH groups ,RAVmax ,RAVmax/BSA ,RAVpre‐a and RAVpre‐a/BSA were increased in ND‐CH ,D‐( CH+EH) groups ,RAVmin and RAVmin/BSA were increased in D‐( CH+ EH ) group ( P <0 .05) . Compared with control ,NG ,CR ,ND‐EH and ND‐CH groups ,RAVpre‐a and RAVpre‐a/BSA were increased in D‐( CH + EH ) group ( P < 0 .05 ) . ② RA reservoir function parameters :compared with control ,NG and CR groups ,RA TotEV was increased in ND‐CH and D‐( CH+ EH ) groups ( P <0 .05 ) . Compared with control ,NG ,CR and ND‐EH groups ,RA TotEV was increased in D‐( CH + EH ) group ( P<0 .05) . ③ RA conduit function parameters :compared with control ,NG and CR groups ,RA PassEF was decreased in ND‐EH ,ND‐CH and D‐( CH + EH ) groups ,RA PassEV was decreased in ND‐CH and D‐( CH+EH) groups( P <0 .05) . Compared with control ,NG ,CR and ND‐EH groups ,RA PassEV was decreased in D‐( CH+EH) group ,RA PassEF was decreased in ND‐CH and D‐( CH+ EH ) groups ( P <0 .05) . ④ RA pump function parameters :compared with control ,NG and CR groups ,RA ActEF were increased in ND‐CH and D‐( CH+EH) groups ( P <0 .05 ) ; Compared with control ,NG ,CR and ND‐EH groups ,RA ActEV was increased in ND‐CH and D‐( CH+EH) groups ( P <0 .05) . Conclusions Different left ventricular geometric patterns have different RA structure and function in OSAS ,patients with ND‐CH and D‐( CH + EH ) have worse RA structure and function . T he results of this study suggested that the clinical need to pay attention to the RA structure and function of OSAS patients with dilated LV H .

ABSTRACT

Objective To investigate the clinical features of elderly patients with stage 3-4 chronic kidney disease ( CKD ) and to analyze the risk factors of the kidney function progression. Methods This was a cross-sectional study. The clinic data of elderly patients (≥60 years) with stable clinical manifestation in Beijing Hospital from January ,2014 to December ,2015 was collected.Based upon the eGFR derived from Cockcroft-Gault (CG) equation ,all patients were divided into groups of CKD 3a group ,CKD 3b group and CKD 4 group ,respectively. Moreover ,patients were separated into the old group (< 80 years) and the very old group (≥80 years) based on the age. The demographic features ,primary diseases history ,body mass index (BMI) ,blood routine ,urine routine , blood biochemistry ,Cystatin C and iPTH were recorded.Logistic regression analysis of kidney function progression was also conducted. Results One-hundred and eighty-three cases were enrolled with a mean age of(80 ± 9)years.Along with the descent of kidney function ,the age ,levels of proteinuria and iPTH gradually ascended (F= 12.352 ,5.910 ,5.910 ;P= 0.000 ,0.003 ,0.003 ,respectively) and the BMI and serum albumin gradually descended (F=9.758 ,11.088 ;P=0.000 ,0.000 ,respectively).In addition ,compared to the old group ,very old group was associated with decreased level of eGFR [(37.1 ± 9.9) ml·min-1·1.73 m -2vs. (44.0 ± 12.0) ml·min-1·1.73 m -2,t= -4.280 ,P=0.000] and increasedlevelofCystatinC[(1.71±0.84)mg/L vs. (1.40±0.69)mg/L ,t=3.484 ,P=0.001].The Logistic regression analysis indicated that the proteinuria was an independent risk factor for the progression of kidney function ( OR =3.856 ,P=0.004). Conclusions As the gradually descended of the eGFR level in elderly patients with stage 3-4 CKD ,age ,proteinuria and iPTH gradually ascended ,BMI and serum albumin descended ,respectively. The level of eGFR decreased and CystatinC increased significantly with the increasing age. Proteinuria was an independent risk factor for progression of the kidney function.

ABSTRACT

Objective To investigate the value of hydrogen proton MR spectroscopy (1H-MRS) and Dixon sequence for the quantitative diagnosis and classification of steatosis in patients with non-alcoholic fatty liver disease (NAFLD). Methods Sixty seven patients with NAFLD confirmed by liver biopsy were prospectively collected from October 2015 to May 2017 in Hangzhou Normal University Hospital. All patients underwent 1H-MRS and Dixon sequence scan within 7 days after liver biopsy, 1H-MRS-based hydrogen proton density fraction (MRS-PDFF) and Dixon-based hydrogen proton density fraction (MRI-PDFF) were obtained. Fat grading based on the fat percentage obtained from liver biopsy. Pearson correlation analysis was performed to analyze the correlation among pathological steatosis and MRS-PDFF, MRI-PDFF. One-way ANOVA analysis was performed to compare the difference of PDFF between patients with different degrees of severity of fatty liver. And the ROC curve analysis was performed to generate the thresholds of MRS-PDFF and MRI-PDFF for determining the presence of fatty liver. Results The steatosis grade of pathological biopsy showed grade S1 in 36 cases, grade S2 in 16 cases, grade S3 in 15 cases, the MRS-PDFF values of S1, S2 and S3 patients were (8.25 ± 4.32)%, (15.67 ± 4.54)%, (23.46 ± 5.82)%and the MRI-PDFF values were (6.31 ± 2.94)%, (15.42 ± 5.07)%, (24.47 ± 6.31)%. Statistically significant differences were observed among them (P<0.01). Both MRS-PDFF and MRI-PDFF were positively correlated with histological fat percentage (r values were 0.840 and 0.892,all P<0.01), there was also a correlation between MRS-PDFF and MRI-PDFF (r=0.930, P<0.01). Area under ROC curve of MRS-PDFF and MRI-PDFF for differential diagnosis of grade S1 steatosis were 0.955 and 0.976, and area under ROC curve for differential diagnosis of grade S3 steatosis were 0.972 and 0.978. Conclusion 1H-MRS and Dixon sequces have high value in liver fat content detection and classification of patients with NAFLD, and both have similar diagnostic efficacy.

Objective To investigate the effect of preoperative anxiety on propofol EC50 for nomovement during Painless gastroscopy.Methods Thirty-one patients (without anxiety) and twenty-seven patients (with anxiety) undergoing gastroscopy under general anesthesia were enrolled on the study.Anesthesia was conducted with a target-controlled infusion (TCI) of propofol.The initial target effect-site propo fol concentration (Ceprop) was 5.0 μg/ml and was adjusted stepwise by 0.5 μg/ml by an up-down sequential method to reach no-movement.Results Propofol EC50 to induce no-movement was higher in patients with anxiety than those without anxiety (5.32 μg/ml,95% CI:4.75-5.88 μg/ml vs 4.75 μg/ml,95% CI:4.48-5.01 μg/ml,P < 0.05).Conclusions During painless gastroscopy,patients with anxiety had a higher propofol EC50 for no-movement compared with those without anxiety when intravenous injected of fentanyl 0.1 μg/kg.

ABSTRACT

Objective To evaluate the feasibility of contrast enhanced ultrasound (CEUS) in gallbladder carcinoma anti-angiogenesis treatment.Methods Subcutaneous gallbladder carcinoma model was consturcted.The mice were divided into intervention-drug group and control group randomly.The mice of intervention-drug group were treated with Endostar (10 mg · kg-1 · d-1) by intraperitoneal injection for two weeks.Two groups of mice were detected by CEUS,then the time of arrival (AT),peak time (TTP),peak intensity (PI),area under the curve (AUC) were measured via time-intensity curve.Expression of MVD and VEGF both in the intervention and control groups were studied through immunohistochemistry.and the correlations between MVD,VEGF and CEUS parameteres were further analyzed.Results The mean values of PI in drug intervention group and control group were 10.8 ± 5.5 and 16.8 ± 5.8,respectively.The values of PI in intervention-drug group were lower than that in control group significantly (P < 0.05).There was no significant difference in AT,TTP,AUC between the two groups.The mean values of MVD on drug intervention group and control group were 8.5 ± 3.8 and 13.1 ± 3.5,respectively.The mean values of VEGF on drug intervention group and control group were 4.3 ± 0.5 and 4.7 ± 0.4,respectively.The values of MVD and VEGF in intervention-drug group were significant lower than that control group (P < 0.05).MVD and VEGF values of intervention-drug group were correlated with PI (r =0.712,P < 0.05;r =0.739,P < 0.05).Conclusion Endostar can inhibit the growth of gallbladder carcinoma and PI can be used as an effective marker to evaluatethe effect of anti-angiogenic therapy in gallbladder carcinoma.

ABSTRACT

Objective To investigate the effects of light acute hypervolemic hemodilution on early postop-erative cognitive function in elderly patients. Methods A total of 60 patients treated by radical surgery were equally randomized to an AHH and a non-AHH control group.Using the Mini-Mental State Examination scale (MMSE),we evaluated the cognitive function of the patients.We made comparisons between the two groups in the plasma S100β levels at T0(before anesthesia induction),T1(immediately after hemodilution),T2(immediately after operating),T(36 hour after operating)and T(424 hour after operating).Results The S-100β lever at T2、T3 and T4were markedly higher than T0in both groups(P<0.05);At T3and T4,the S-100β lever was higher in non-AHH control group than AHH group(P<0.05).There was no significant difference in the incidence of postopera-tive cognitive dysfunction between the two groups(P>0.05)There was no significant difference in the incidence of postoperative cognitive dysfunction between the two groups(P > 0.05). Conclusions AHH can significantly reduce plasma S100β in elderly patients,but there is no effect on early postoperative cognitive function.

Objective To explore the correlation between CEUS blood perfusion characteristics and angiogenesis and cell proliferation in nude mice models of gallbladder carcinoma transplant tumor.Methods Nude mice models of gallbladder carcinoma transplant tumor were established.The blood perfusion characteristics of the nude mice models were quantitatively analyzed,CEUS quantitative parameters and the expression levels of microvessel density (MVD),vascular endothelial growth factor (VEGF) and proliferating cell nuclear antigen (PCNA) immunohistochemically were obtained.The correlations were performed.Results Twenty tumors were successfully established.The CEUS of 16 tumors was successful.There was positive correlation between peak intensity (PI) and MVD,PCNA (r=0.635,0.514,P=0.008,0.042).The CEUS parameters were not correlated with expression of VEGF.MVD had positive correlation with expression of VEGF,PCNA (r=0.680,0.650,P=0.004,0.006);VEGF had positive correlation with expression of PCNA (r=0.611,P=0.012).Conclusion The PI of CEUS is positive correlation with MVD and PCNA in nude mice models of gallbladder carcinoma transplant tumor,which may be helpful to evaluate tumor angiogenesis and cell proliferation state.The MVD,VEGF and PCNA positively correlates to each other,which indicates there is the relationship of reciprocal promotion between tumor angiogenesis and cell proliferation,and is good for tumor growth.

Objective@#An innovative technique was established to rapidly construct various cell lines that could be induced to express multiple influenza A virus (IAV) proteins.@*Method@#The HA protein genes of multiple IAVs were cloned into the Cumate-induced expression system which was positioned between two PiggyBac transposon sites. These HA plasmids were transfected into the HEK293A cell line with the PiggyBac transposase plasmids. The transfected cells were screened with puromycin, and after that the corresponding virus proteins were induced with Cumate.@*Results@#The results of flow cytometry and Western blotting showed that the virus proteins were expressed in most of the cells in corresponding lines after the induction of Cumate.@*Conclusion@#Cell lines which were inducible to express IVA HA protein can be efficiently constructed by using the PiggyBac transposon system.

ABSTRACT

Objective To prospectively investigate the characteristics and correlative influential factors for the changes in pulmonary artery hypertension (PAH)in chronic kidney disease (CKD) patients without end-stage renal disease.Methods Complete clinical and laboratory data of 133 outpatients without ESRD (CKD stage 2,3 and 4)in Beijing Hospital between January 2012 and January 2013 were retrospectively analyzed.Pulmonary artery systolic pressure(PASP)was assessed by echocardiography according to the guideline from the American Society of Echocardiography in 2010,and PASP more than 35 mmHg(1 mmHg=0.133 kPa)was diagnosed as PAH.Results PAH was found in twelve(9.0 %) patients,including 7 females and 5 males,with mean age(82.9 ± 5.4 years).The prevalence of PAH was 2.3%,10.9% and 17.6% in the patients of CKD stage 2,3 and 4(P< 0.05) respectively.PASP could be calculated in 67 patients in whom tricuspid regurgitation was present.The mean PASP was (27.1 ± 5.4) mmHg,(31.0 ± 8.3) mmHg and (37.3 ± 13.8) mmHg in patients with CKD stage 2,3 and 4,respectively(P < 0.05).Body mass index(BMI),eGFR and BUN showed significant differences between PAH group and non-PAH group with statistical significance (P<0.05).Multivariate logistic regression analysis revealed that the lower BMI and the higher stage of CKD were independent risk factors for PAH (P < 0.05).Conclusions PAH is not a rare complication in the CKD patients without end-stage renal disease.The incidence of PAH is significantly associated with lower BMI and the higher stage of CKD.Doppler echocardiography should be performed periodically to detect PAH in CKD patients in order to improve the prognosis of these patients.

ABSTRACT

Objective To explore the differences in clinical and pathological characteristics of idiopathic membranous nephropathy between elderly and non elderly patients.Methods The clinical and pathological data,which were used for biopsy diagnosing idiopathic membranous nephropathy in elderly patients (aged ≥65 years) versus non elderly patients (aged <65 years) in our hospital during the recently 10 years,were retrospectively analyzed and compared.Results The elderly patients with idiopathic membranous nephropathy accounted for 26.29% and patients aged less than 45 years with idiopathic membranous nephropathy accounted for 25.26%.The proportions of idiopathic membranous nephropathy in the two different age groups were similar.The occurrence rate of hypertension and microscopic hematuria as well as levels of blood serum creatinine and urinary N-acetyl-β-D-glucosaminidase were higher,while the urinary osmotic pressure,estimated glomerular filtration rate (eGFR) and blood albumin were lower in elderly group than in non-elderly group (all P<0.05).The proportions of glomerulosclerosis,tubulointerstitial lesions and vascular lesions were higher in elderly group than in non-elderly patients (all P< 0.05).There were no significant differences in the immunofluorescence intensity of IgG,positive rates of IgA,C3,C4 and Fi between the two groups,while the positive rate of C1q was lower in elderly group than in non elderly.Among 51 patients receiving IgG4 immunohistochemical staining,the positive rate of IgG4 staining and the intensity were higher in elderly group than in non-elderly [100.00% vs.81.25%,(2.11±0.46) vs.(1.59 ± 0.95),both P < 0.05].Conclusions The proportion of idiopathic membranous nephropathy is increased in non-elderly patients.Elderly patients are more susceptible to hypertension,hematuria and renal dysfunction.The positive rate and the intensity of IgG4 are higher and the positive rate of C1q is lower in elderly group than in non-elderly group,which indicate that patients with negative IgG4 and positive C1q have secondary causes which have not yet been demonstrated,and they should be closely observed during the follow-up.

ABSTRACT

ObjectiveTo evaluate the curative effect of supplementedGanmai-Dazaodecoction for malignant tumor correlation with depression.MethodsA total of 98 patients with malignant tumor-related depression were divided into a treatment group and acontrol group according to the random number table. The control group were treated with flupentixol and melitracen tablets (Deanxit) combined with conventional treatment, the treatment group were treated with oral administration of traditional Chinese medicine supplemented Ganmai-Dazaodecoction combined with conventional treatment for 8 weeks. The Hamilton Depression Scale (HAMD) was used to evaluate the curative effect.ResultsThe effective rate showed no significant difference between the treatment groupand the control group (79.59%vs.75.51%;χ2=0.059,P=0.809). HAMD scores in both groups were significantly decreased after the treatment than before treatment (12.8±3.4 vs. 29.4±4.7 in the treatment group, 12.3±3.6vs.28.7±4.5 in the controlgroup, allP<0.05), there was no significant difference in HAMD scores after the treatment between the treatment group and the control group (t=0.707,P=0.481). ConclusionCurative effect of supplementedGanmai-Dazaodecoction is equal to flupentixol andmelitracen tablets in patients with malignant tumor-related depression.

ABSTRACT

Objective To comparative analysis the rabbit VX2 tumors angiogenesis in fatty liver and normal liver,and investigate the correlation of contrast-enhanced ultrasound (CEUS) parameters and their angiogenesis.Methods Rabbit models of fatty liver and normal liver with implanted VX2 tumors were established.Two groups of hepatic backgrounds and VX2 tumors were analyzed by QontraXt quantitative software of CEUS.The expression level of tumor microvascular density (MVD) and vascular endothelial growth factor (VEGF) were detected with immunohistochemical techniques and real-time fluorescence quantitative PCR (FQ-PCR).Results ①No significant difference was found in MVD,VEGF protein and gene expression between VX2 tumors in normal liver and fatty liver.The expression of VEGF gene in fatty liver parenchyma were lower than normal liver parenchyma (P <0.05).②There was positive correlation between MVD and peak intensity (r =0.494,P <0.05; r =0.591,P <0.01) both in fatty liver and normal liver VX2 tumors.The expression of VEGF protein were not correlated with TIC parameters (all P > 0.05).③The MVD had positive correlation with expression of VEGF protein in fatty liver and normal liver VX2 tumors (r =0.508,P <0.05; r =0.570,P <0.01).Conclusions The expression of MVD and VEGF had no significant difference between fatty liver and normal liver VX2 tumors.The peak intensity of VX2 tumors CEUS had positive correlation with MVD both in fatty liver and normal liver,which can indirectly reflect MVD expression level and help to evaluate tumor angiogenesis.

ABSTRACT

Objective To improve standards of diagnosis and therapy for acute focal bacterial nephritis bY comparing the characters of acute focal bacterial nephritis and acute pylonephritis.Methods Thirty-five patients of upper urinary tract infection whoever accepted ultrasonographic and computed tomographic (CT) examinations in Beijing Hospital from January 2007 to January 2013 were studied retrospectively.Eighteen patients were diagnosed as acute focal bacterial nephritis (AFBN) according to CT imaging features,the other 17 patients were diagnosed as acute pyelonephritis (non-AFBN).The clinical feature,laboratory and imaging examination were compared between the two groups.Results Leukocyte count,percentage of neutrophil,fasting blood-glucose,ESR,NAG/Cre of urine,the combination of more than one kind of antibiotics and duration of antibiotic treatment were higher or much more in AFBN group patients than non AFBN patients,the differences between two groups were statistical significance(P < 0.05).Urine microorganism culture was mainly Escherichia coli no matter in AFBN group or non-AFBN group,microorganism species of AFBN patients were more complex than non-AFBN patients.The most common contrast-enhanced CT feature of AFBN included local distribution of inflammatory area,wedge-shaped or unregular-shaped hypodense lesions on bilateral or single kidney,and mass-like hypodense lesions in more severe cases.NAG/CRE of urine was higher in AFBN group patients with bilateral kidney lesions than with single kidney lesions and non AFBN patients.The remission time of AFBN and non-AFBN was (3.4 ± 2.9) d,(3.0 ± 1.8)d respectively.The relapsing rate of AFBN and non-AFBN within half a year was 2/18,2/17,respectively.Conclusions The symptoms of acute focal bacterial nephritis are not specific.Abdominal contrast-Enhanced CT,ESR,leukocyte count,urine NAG/CRE are probably a predictor of AFBN.Antibiotic treatment with appropriate usage and sufficient duration could bring satisfactory outcomes and prognosis in acute focal bacterial nephritis and pyelonephritis patients.
